Many healthcare workers understand that the health of their patients is related to conditions in their lives, known as social determinants of health. These conditions existed long before any illness – in their homes, schools, jobs and neighborhoods. The need to address social determinants of health is especially urgent for health systems serving minority and low-income populations and nonprofit hospitals, which are required to demonstrate “community benefit,” i.e., how they contribute to the health of the communities they serve.

In this webinar, we invite you to participate in a conversation with Dr. Sanne Magnan about what you can do to address the social determinants of health and to advance health equity in your communities.

Dr. Magnan has written several articles on healthcare systems and social determinants of health. She is the previous co-chair of the Roundtable on Population Health Improvement of the National Academies of Sciences, Engineering and Medicine (NASEM). Currently, she is co-chair of the Health Care Expenditure Collaborative associated with the Roundtable.

In 2007, she was appointed Commissioner of the Minnesota Department of Health (2007-2010) and had significant responsibility for implementation of Minnesota’s 2008 health reform legislation, including the Statewide Health Improvement Program (SHIP), standardized quality reporting, development of provider peer grouping, certification process for healthcare homes, and baskets of care. She is a Senior Fellow with HealthPartners Institute, holds a PhD in medicinal chemistry and an MD from the University of Minnesota, and is a board-certified internist.
